Putnam County 4-H Enrollment Options

Online enrollment is no longer available as of (4/2/2013).  If you still wish to enroll in projects other than archery, dogs, horse and pony, and Crossbred Heifers Steers, you can download the paper form and contact the Extension Office at (765)653-8411.

One may enroll into the Putnam County 4-H Program when in grades 3-12. There are two options to enroll. One may enroll online below or via the traditional paper form that is downloadable below or available at the office. Also note that if you are enrolling into an animal project, there are additional animal enrollment forms that must be submitted by the proper deadlines. These are available at the Extension Office or often from your club leader.

Fees for 2013 will remain the same as in 2012. Of the $25 per child program fee, $15 per youth is the state program fee which will go to Purdue University College of Agriculture 4-H Youth Department to improve service from the state level back to our county. The other $10 goes to the Putnam County 4-H Council for local program costs. Manuals which are ordered by the local Extension office from Purdue and used multiple years are $5 each. There is no additional charge for manuals that are internal to Putnam County that are produced locally.

Make checks payable to the Putnam CES Ed Fund if mailing or presenting checks in the office. Office staff will see that the portion owed to the state is submitted to Purdue University and the portion to reside in the county is turned over to the Putnam County 4-H Council.
