Adult Volunteer Application: Return to the Extension Office in order to begin your approval process.  This is required of all adults working with children in the Vanderburgh County 4-H Program.

Volunteers are required to read through the training  Powerpoint and Handout before their approval process can be completed.

Trip Reimbursement Form: Return to the Extension Office.

Youth Council Grant Application: Use this form to request a Youth Council grant for your club.

Accident/Incident Report
